All fees need to be paid in advance of services provided following the signing of a Service Agreement.
Comprehensive evaluation of the Client (including a Safety Evaluation of the Home and a Physical/ Mental/ Emotional/ Spiritual Assessment) , Medication Review, AND development of an Individualized Action Plan (also know as a Care Plan); approx. 3-5 hours
Regular weekly 1-hour visit to evaluate Client's status/ Education and support of caregivers/ Advocacy with healthcare team on Client's behalf/ Connecting Client to community resources/ Discussion of placement options/ Discussion of legal documents (POA-HC, Living Will, 5 Wishes, DNR/POLST)/ Support with end-of-life issues/ help w ith hospice arrangements/ Follow-up phone call to family (POA-HC)
Initial 15-minute phone consultation to review what your goals and needs are, the range of services available to you, & development of a basic plan of action.
Above services as well as: getting prescriptions refilled/ organizing pill boxes, monitoring for side effects of medications/ arranging with MD for new orders or changes in orders/ arranging delivery of medications
For additional time spent on home visits requiring assessment skills/ for complex discussion and planning with Client's physician and healthcare team/ Tours of AL/IL/Memory Care facilities/ Assistance with visits to the doctor's office, therapy sessions
